Notification Methods
The Hopper Group may contact our registered patients via SMS and email to provide notifications and secure access to online forms (also noted is the SMS notification language):
- Appointment Reminders and Confirmations: “Your appointment at The Hopper Group is on 01/31/2026 at 8:00 AM. Reply ‘Y’ to confirm or call 817.274.8800 to reschedule. Reply STOP to opt out.”
- Patient Surveys: “Please complete and submit the [form name] form sent by The Hopper Group. Reply STOP to opt out.”
- Patient Notifications: “Please review the [form name] form sent by The Hopper Group. Reply STOP to opt out.”
Participation
Participation in SMS and email communications is voluntary and not a requirement for joining the practice. Consent is provided by checking the box on the online form titled “Patient Communications Agreement,” which serves as a standalone, affirmative opt-in action. The Hopper Group sends this consent to all registered patients.
Please note that this consent form is not publicly available on our website and is separate from any other consent required to join the practice.
Message and data rates may apply. Carriers are not liable for delayed or undelivered messages.
Record Keeping
Each signed Agreement is retained by The Hopper Group for compliance purposes.
The initial SMS will state: “This is The Hopper Group; you are subscribed to receive SMS notifications. Reply ‘STOP’ to opt out.” All subsequent messages will include: “Reply ‘STOP’ to opt out.”
Messaging Opt-In/Opt-Out methodologies and communication detail
- Opt-In Process: To receive SMS and email messages from The Hopper Group (THG), please complete the following steps on the Patient Communications Agreement consent:
- Provide your preferred phone number and email address for secure communication.
- Check the box to confirm your agreement to receive SMS and email notifications.
- Sign the document to acknowledge your understanding and consent.
- Opt-Out Process: You may opt out of SMS communications at any time by replying STOP to any message. Upon opting out, you will receive: “You have successfully unsubscribed. You will not receive any more messages from The Hopper Group.”
- HELP Message: For assistance reply HELP to any message. You will receive: “The Hopper Group: For help, call our office at 817.274.8800. Reply STOP to unsubscribe.”
- Message Frequency: Message frequency may vary based on communication type and appointment frequency. You may receive up to 4 SMS text messages per week (up to 2 for appointment reminders and up to 2 for other notifications).
- Privacy Policy:Data collected from communications or information submissions through our secure online forms is considered part of an individual’s medical record and is used to facilitate the agreed-upon medical care. All patient information is subject to our health information practices policy. We do not share or sell your email address, mobile number, or personal information to third parties for marketing purposes. Your information is used solely for healthcare-related communications as outlined above.
We take strong precautions to protect your information, including encryption and access controls. While we strive for security, please note that no internet transmission method is completely secure.
